However, when resident Hongsik went missing, his wife took the villagers and the Public Health Center staff to find him. Changmok got infuriated because everyone walked out of the ceremony while he was giving a speech in front of the press. Naturally, he fired Byeongcheol for not being able to control the residents and restricted him from ever entering his office. Unable to hold it any longer, Byeongcheol barges into the Public Health Center, blaming the staff for causing his downfall. Hwang tries to calm him down but he refuses to acknowledge his mistake of making a career out of flattering Changmok. Byeongcheol’s frustration hits the ceiling when Chiyeon tells him to calm down because of their past history. Shouting and cursing at everyone, Byeongcheol dislocates his jaw twice, only for Jiui to pop it back in. Byeongcheol finally stops throwing a fit and leaves the Public Health Center because he is ashamed.

Why Does Chiyeon Report Hwang’s Malpractice?Jiui and Hari get urgently summoned for treating a sick senior resident at Jineondo. After arriving there, when Hari goes to set up an IV bag, she finds an injection wound on the elderly woman’s hand. Although both Jiui and Hari don’t give it much thought after the woman claims she got an IV shot earlier, things take a turn for the worse when multiple elderly women fall ill at night. Hari finds injection wounds on all of their hands and Jiui gets to know that they were given the shots by a retired nurse, Ms. Hwang. After meeting Hwang and inspecting the IV bags in her refrigerator, Jiui finds that the nurse unknowingly stored expired medicines. She initially thought they were clumpy because of the cold weather, but when she realized that was not the case, it was too late already. Jiui honors the local’s request and lets Hwang go, knowing she is the only source of medical care on the island. However, Chiyeon, who visits Jineondo to deliver emergency medicines, reports Hwang to the police. He knows that if Hwang continues her malpractice, something fatal is bound to happen and the blame will fall on the Pyeondongo clinic staff.
What Causes Jiui To Lose His Confidence? One night, upon receiving a patient with severe abdominal pain, Jiui finds his symptoms similar to that of appendicitis. Since it requires surgery, he asks Hari to look for a ferry to the mainland but unfortunately, bad weather has grounded the ferries. When Hari calls for a chopper, it crashes while landing on the newly constructed helipad, instantly killing the pilot. The incident leaves Jiui and Hari in shock. Later, when the man’s pain turns out to be a normal stomach upset, Governor Changmok calls out the young doctor for improper diagnosis. The dead pilot’s family blames Jiui for the death, which reminds him of the time he got accused of letting his friend Seonu die. The media tears Jiui apart, labeling him as guilty and claiming his alleged misdiagnosis cost the pilot’s life. Jiui loses his confidence as a doctor and as days go by, he isolates himself inside a room, ignoring everyone including Hari. Things become complicated as ER Doctor Hwayeng arrives at Pyeondongdo – she used to date Jiui and was friends with Seonu. Hwayeng broke up with Jiui just after Seonu’s accidental death and did not care about the young doctor’s feelings. Jiui feels agitated at the sight of Hwayeng, and requests her to leave but she doesn’t. It’s particularly difficult for Jiui because Hwayeng left him in the hardest phase of his life. Meanwhile, as news of the crash spreads, arriving patients refuse to get treated by Jiui.
Hari and Chiyeon Helps Jiui Get Over His PastChiyeon deeply cares about Hari’s happiness because he still likes her. Upon noticing a look of confusion and jealousy on Hari’s face whenever Jiui talks with Hwayeng, Chiyeon decides to eliminate the chances of a misunderstanding. He encourages Hari to visit Jiui’s room with food since the doctor has not eaten for long. Hwayeng fails to dissuade Hari from helping Jiui by talking about the doctor’s fear of staying in islands since Seonu’s death. Upon hearing that another island had a helicopter crash because the helipad was not lit, Hari and Chiyeon team up to investigate. Thankfully, Hari remembers that the Pyeondongdo helipad also had no lights while the chopper was about to land, leading to the crash. The helipad issues stem from Governor Changmok, who replaced well-built pads with rushed faulty construction to win the election. After Hari and Chiyeon complete their investigation in all nearby islands, the latter sends a notice to the press for defaming Jiui.
Jiui’s World Gets Shattered Despite Hari’s HelpJiui feels grateful after learning how Hari visited all islands and inspected helicopter crashes just to save his name. He never thought Hari would be so dedicated to saving him from getting defamed. Jiui gets his confidence back and finally prepares to resume his duty as the Public Healthcare doctor, leaving Hari and the staff happy. While Eom waits for the revised news articles to pop up, he notices a major headline that claims Jiui misdiagnosed because of his deteriorating mental health. Allegedly, the medications Jiui uses to control his panic attack and anxiety are responsible for his impaired judgement. Jiui fails to take it anymore as his career seems to be over. He soon breaks down, and his hands start trembling as he gets flashbacks from Seonu’s tragic death. Suddenly, the fear of living on islands strikes his mind and Jiui runs upstairs in his room. By the time Hari steps inside, Jiui has given up on everything – he just wants to get off Pyeondongdo at all costs. Hari consoles Jiui and hugs him as he breaks down in tears but will she be able to save him from the tragedy? We will find the answers in the final two episodes.
